Care Monitoring and Coordinating Services: 
These services are provided by a LifeCompass Care Manager, billed on an hourly basis and can include:

• In-home monitoring
• Nursing home monitoring
• Retirement home assessment and/or monitoring
• Travel or relocation arrangements
• Scheduling of medical appointments
• Accompaniment on appointments
• Referral, coordination and supervision of home safety modification(s)

Referral and Community Resources:
LifeCompass Care Manager can provide referral and information on services needed by a senior.
These can include:

• Personal Bill paying assistance
• Government agencies and programs
• Elder Law attorneys
• Home maintenance
• Transportation options
• Moving Assistance
• Medication options
